支撑喉镜下声门暴露困难综合预测系统的建立

Study of building up a comprehensive system for predicting difficult laryngeal exposure under suspension laryngoscopy

  • 目的:建立简便有效的支撑喉镜下声门暴露困难综合预测系统。方法:选择120例声门良性肿物切除手术患者,应用Logistic回归分析支撑喉镜下声门暴露困难的各种影响因素,包括体重、身高、BMI、颈围、颈部后仰角、最大张口度、颏至甲状软骨间距离、颏至胸骨上窝间距离、Mallampati分级、Yamamoto分级、咬上唇试验分级。建立数据库,结合患者术中声门暴露情况,通过统计学分析筛选上述相关影响因素,构建支持喉镜下声门暴露困难综合预测系统。结果:声门暴露困难相关因素为BMI、颈围、最大张口度、颈部后仰角、颏至甲状软骨间距离、Mallampati分级和Yamamoto分级(均P<0.05);将已得出的7项相关因素作为预测指标,每项分别设1、2、3分,总分21分,分数越大,发生声门暴露困难的危险性越大。预测系统设定>8分为预测暴露困难,实验获得预测系统(>8分)的灵敏度、误诊率、漏诊率和特异度分别为95.5%、5.3%、4.5%、94.7%。结论:说明支撑喉镜下声门暴露困难综合预测系统可为术前识别困难患者提供依据。
