摘要: 目的:利用荟萃分析系统性评价循环microRNA对鼻咽癌的诊断价值。方法:检索国内外多个数据库,收集从各数据库建库之日至2018年7月公开发表的关于microRNA用于鼻咽癌诊断的临床研究。根据排除和纳入标准进行文献的筛选、资料的提取和文献质量的评估,然后运用Meta-disc和Stata等软件对最终的纳入研究进行Meta分析。18篇符合条件的文献被收集用于系统性回顾,其中6篇文献共7个独立的病例对照研究,累计706例鼻咽癌患者和353例对照者被纳入用于Meta分析。因各研究之间存在较明显的异质性,故选择运用随机效应模型进行评价分析。循环microRNA用于诊断鼻咽癌的合并灵敏度、特异度、阳性似然比、阴性似然比、诊断比值比及其95%可信区间(95%CI)和曲线下面积分别为0.78(0.75~0.81)、0.81(0.77~0.85)、5.71(2.49~13.11)、0.23(0.13~0.42)、30.88(7.91~120.59)、0.913 1,说明microRNA用于诊断鼻咽癌具有较高的诊断准确性和较大的临床实用性。亚组分析结果显示,样本来源、病例数、检测miRNA数目和对照组类型均是引起异质性的重要原因。结论:利用循环microRNA诊断鼻咽癌具有重要的潜在价值。

Abstract: Objective: To evaluate the diagnostic value of circulating microRNAs in nasopharyngeal carcinoma by Meta-analysis. Method: A number of databases in Chinese and English were searched, and the clinical studies published from the day the datebases were set up to July 2018 on detecting microRNAs as biomarkers in the diagnosis of nasopharyngeal carcinoma were collected. Literature selection, data extraction and literature quality evaluation were carried out according to the criteria of exclusion and inclusion.Then Meta-disc,Stata,and other softwares were used for the Meta-analysis.Result: Eighteen eligible articles were collected for systematic review, of which 7 independent case-control studies from 6 articles including 706 NPC patients and 353 controls were selected in the Meta-analysis. Since the obvious heterogeneity among the studies was observed,we conducted this Meta-analysis by the random effect model. The pool sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and area under curve of circulating microRNAs in the diagnosis of nasopharyngeal carcinoma were 0.78(95%CI: 0.75-0.81, 0.81(95%CI: 0.77-0.85), 5.71(95%CI: 2.49-13.11), 0.23(95%CI: 0.13-0.42), 30.88(95%CI: 7.91-120.59) and 0.913 1,which illustrated the high diagnostic accuracy and significant clinical utility of microRNAs in the diagnosis of nasopharyngeal carcinoma.Subgroup analysis showed that the source of samples,the number of cases,the amount of the detecting microRNAs and the type of control groups were the main reasons contributing to the significant heterogeneity. Conclusion: MicroRNAs as biomarkers in the diagnosis of nasopharyngeal carcinoma possess significant potential value.-
