Preliminary experience of transoral robotic surgery in laryngeal and hypopharyngeal tumors with da Vinci surgical system

摘要: 目的:总结达芬奇机器人手术系统经口切除喉及下咽部肿瘤的临床经验。方法:回顾分析2016-07-01-2018-01-31期间首都医科大学附属北京安贞医院耳鼻咽喉头颈外科共12例达芬奇机器人经口切除喉及下咽部肿瘤患者的临床资料。其中喉肿瘤7例,下咽肿瘤5例。结果:男10例,女2例。年龄30~81岁(中位年龄58岁)。12例均用达芬奇机器人经口入路完全切除,无中转开放病例。机器人平均手术时间为31.7 min,平均出血量为12.3 ml,平均经口进食时间为4.6 d。术后切缘均为阴性,同期行颈部淋巴结清扫8例,术后辅助放疗7例。术中术后无严重并发症出现。术后随访3~20个月,1例颈部淋巴结转移,无局部复发、远处转移或死亡病例。结论:在经选择的病例中,达芬奇机器人经口切除喉及下咽肿瘤是一种安全可行的微创外科方法。Abstract: Objective: The purpose of this study was to summarize preliminary clinical experience of transoral robotic surgery (TORS) in laryngeal and hypopharyngeal tumors with da Vinci surgical system.Method: Twelve patients were reviewed who were diagnosed with laryngeal (n=7) and hypopharyngeal tumors (n=5) and treated with TORS in Beijing Anzhen Hospital from July 1, 2016 to January 31, 2018.Result: Ten cases were male and two cases were female. The median age was 58 years old (ranging from 30 to 81 years). The robotic surgeries were performed successfully on all of the 12 patients. The average surgical time was 31.7 min. The average blood loss was 12.3 ml. The average recovery time for oral intake was 4.6 day. All surgical resection margins were free of carcinoma. Eight cases received neck dissection and 7 cases received adjuvant radiotherapy. There were no perioperative complications. There was no local recurrence, metastasis or death except 1 regional recurrence with a follow-up of 3 to 20 months.Conclusion: TORS is a feasible, safe, minimally invasive, and effective surgical procedure in selected laryngeal and hypopharyngeal tumors.
