Study on the ultrastructural pathological change of nostril mucosa after adenoidectomy in children with temperature-controlled coblation

摘要: 目的 探讨应用低温等离子治疗儿童腺样体肥大后鼻孔黏膜超微结构的变化特点, 为临床手术避免损伤后鼻孔黏膜提供依据。方法 选取50例行低温等离子腺样体消融术患儿后鼻孔黏膜, 另外选取10例腺样体术后0.5~2年患儿的后鼻孔黏膜作为对照, 所有标本行透射电镜检查, 根据电镜结果明确后鼻孔黏膜即时损伤超微结构特点及愈合后黏膜超微结构特点。结果 即时损伤黏膜超微结构明显破坏, 愈合后黏膜为假复层纤毛柱状上皮, 纤毛内"9+2"小体形成不完整, 纤毛功能受到影响。结论 应用低温等离子行小儿腺样体手术过多损伤后鼻孔黏膜, 会对鼻腔黏液毯功能造成影响。Abstract: Objective To explore the characteristics of ultrastructural pathological changes of the choanal mucosa after adenoidectomy in children with temperature-controlled coblation. So as to provide evidence to avoid surgical injury in choana.Method 50 choanal mucosa samples were collected from children with adenoidectomy using temperature-controlled coblation; another 10 samples from patients 0.5-2 years after surgery were taken into account as control. All specimens were examined by transmission electron microscopy (TEM). The ultrastructural characteristics of the immediate injury and healing mucosa of the choanal mucosa were determined according to the results of the TEM.Result The ultrastructural damage of mucosa were obvious in the immediate injury; the healed mucosa was covers with pseudostratified ciliated columnar epithelium. Due to the incomplete formation of "9+2" bodies in the cilia, the ciliary function is weakened.Conclusion The application of temperature-controlled coblation in adenoidectomy of children has excessively damaged the nostril mucosa, which will affect the function of the nasal mucus blanket.
