  • 目的:研究人工耳蜗患儿进入正常学校后听觉语言能力以及在校生活情况,评估手术对患儿及其家庭的长远综合助益。方法:参考听觉行为分级标准(CAP)、语言可懂度分级标准(SIR)及疾病家庭负担量表编制问卷表,问题涉及一般情况、听觉语言能力、在校情况、家庭情况4个方面,选择使用人工耳蜗至少2年并已进入正常学校学习的患儿作为研究对象,对其家长进行问卷随访,共收到50份有效问卷。以一般情况中性别、手术时年龄、父母学历、居住地、人工耳蜗使用时间、家长平时陪伴患儿时间为变量分组,比较各组在听觉语言能力和在校情况的差异,同时比较15项家庭情况指标在手术前后的差异。结果:各变量因素在听觉语言能力的组间比较中均差异无统计学意义(P>0.05);在校情况的比较中,以陪伴时间分组时,交友数量差异有统计学意义(P<0.05),其余各项因素的组间比较均差异无统计学意义(P>0.05);手术前后各项家庭情况的比较中,除经济负担、父母社交时间差异无统计学意义外(P>0.05),其余各项指标均差异有统计学意义(P<0.05),总体表现为术后较术前明显好转。结论:在各变量因素不同的情况下,经术后2年左右的康复,患儿均能够获得较好的听觉语言能力,可以适应在校生活、完成学业,但与同龄健听儿童相比在语言组织能力方面尚有欠缺;此外,父母精神压力随着患儿的康复及入学显著缓解,家庭生活正常化、家庭氛围良好,人工耳蜗不仅使患儿摆脱残疾,对其家庭也有非常明显的助益。
