摘要: 目的: 探讨突聋伴BPPV患者的临床与实验室检查结果的特点。方法: 收集2015-03-2016-03期间在中山大学孙逸仙纪念医院耳鼻咽喉科住院的突聋患者的病史资料、纯音听阈检查结果及视频眼震图检查资料并分析。结果:50%患者曾有眩晕、头晕、平衡障碍等前庭功能损伤的表现,其中16%表现为BPPV;伴BPPV者冷热试验CP异常率为77.8%,非BPPV者 CP异常率为52.9%,无眩晕者CP异常率为32.1%;不伴眩晕的患者,其出现全聋型听力曲线的比例(25.0%)低于伴BPPV的患者(73.9%)及伴非BPPV眩晕的患者(54.2%);伴BPPV者20例,治疗有效率达50.0%(10/20);伴非BPPV眩晕者41例,治疗有效率为36.6%(15/41);无眩晕者55例,治疗有效率为40.0%(22/55)。各组间治疗有效率差异无统计学意义(P>0.05)。结论:约半数的突聋患者曾有眩晕症状出现,16%患者表现为BPPV。伴BPPV的患者其冷热试验异常率高于不伴眩晕的患者;伴BPPV的患者及伴非BPPV眩晕的患者,其出现全聋型听力曲线的比例高于不伴眩晕的患者。是否伴有BPPV对突聋的预后有无影响目前暂无定论,本研究的结果提示BPPV并非判断突聋预后的因素。

Abstract: Objective: To investigate the characteristics of the patients with BPPV accompanied by idiopathic sudden sensorineural hearing loss(ISSNHL).Method: Retrospective analysis of 150 cases of patients with ISSNHL. All data of the cases were collected, including history, pure tone audiograms and VNG tests.Result: 16.0% of all patients had BPPV accompanied by ISSNHL.77.8% of these patients had an abnormal caloric test result. 73.9% of them had a profound hearing loss. The hearing outcome between those with and without BPPV had no significant difference.Conclusion: Patients with BPPV accompanied by ISSNHL seem to have a more severe damage than those without BPPV, while the hearing outcomes seem no significant difference between them.
