Expression of serum YKL-40 in anaphylactic rhinitis patients and association of serum YKL-40 with serum IL-4 and IFN-γ

  • 目的:比较不同类型变应性鼻炎(AR)患者血清中YKL-40的表达水平,分析其与IL-4、IFN-γ的相关性,探讨其在AR发病机制中的可能作用。方法:选取轻度间歇性、轻度持续性、中-重度间歇性、中-重度持续性AR患者各20例作为实验组,正常健康者20例作为对照组。应用酶联免疫吸附法检测各组血清中YKL-40、IL-4、IFN-γ的表达水平。对以上各组数据进行统计学分析。结果:①不同实验组与对照组血清YKL-40相比表达均明显升高,差异均有统计学意义(P<0.05)。轻度持续组与轻度间歇组血清YKL-40相比表达升高,差异有统计学意义(P<0.05)。中-重度持续组与中-重度间歇组血清YKL-40相比表达升高,差异有统计学意义(P<0.05)。在轻度间歇组与中-重度间歇组、轻度持续组与中-重度持续组之间表达差异均无统计学意义(P>0.05)。②不同实验组与对照组血清IL-4相比表达均明显升高,差异均有统计学意义(P<0.05)。各实验组之间血清IL-4表达差异均无统计学意义(P>0.05)。③不同实验组与对照组血清IFN-γ相比表达均明显降低,差异均有统计学意义(P<0.05)。各实验组之间血清IFN-γ表达差异均无统计学意义(P>0.05)。④AR血清YKL-40水平升高与血清IL-4水平呈正相关,与血清IFN-γ呈负相关。结论:①AR血清中YKL-40表达升高,且与IL-4水平呈正相关,与IFN-γ水平呈负相关,提示YKL-40可能通过调节Th细胞的分化,促进Th1向Th2的免疫偏移,参与AR的发病。②与间歇性AR患者相比,持续性AR患者血清中YKL-40表达升高,提示YKL-40可能会促进AR症状的持续发作。
