A new acoustic pharyngometry to evaluate the configuration and functions of pharyngeal cavity in patients with OSAHS

摘要: 目的:采用一种新的声反射咽腔测量方法评估健康成人、单纯打鼾及阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的咽腔结构和功能。方法:采用前瞻性研究设计。对85例正常成人,11例单纯打鼾者及45例OSAHS患者进行声反射咽腔测量,并记录各研究参数,包括口咽结合处距离及面积,会厌游离缘距离及面积,声门处距离及面积,4条曲线的变异系数、咽腔长度、咽腔平均横截面积及咽腔容积。结果:对该方法的重复性评价发现,12 h内的两次测量结果示咽腔长度(Z=-0.881,P=0.378),咽腔容积(Z=-0.196,P=0.844),咽腔平均横截面积(Z=-0.668,P=0.504)差异无统计学意义。正常组与重度OSAHS组之间口咽结合处的面积(χ2=23.453,P=0.000),会厌游离缘的面积(χ2=6.632,P=0.036),声门处的面积(χ2=20.955,P=0.000),4条曲线的变异系数(χ2=7.047,P=0.029),咽腔平均横截面积(χ2=26.923,P=0.000),咽腔容积(χ2=26.529,P=0.000),均差异有统计学意义。BMI、颈围及疾病严重程度分组与口咽结合处的面积、声门处的面积、咽腔平均横截面积及咽腔容积呈负相关。结论:这种新的声反射咽腔测量方法重复性好,并可为OSAHS的诊断甚至分级诊断提供新的临床思路。

Abstract: Objective: To discuss the significance of a new acoustic pharyngometry technique in evaluating the configuration and functions of pharyngeal cavity among health adults, simple snorers and the patients with obstructive sleep apnea hypopnea syndrome(OSAHS). Method: A prospective study was conducted in 85 healthy adults, 11 simple snorers and 45 patients with OSAHS by using a new acoustic pharyngometry technique. The parameters of acoustic pharyngometry, including the distance and the area of the oropharyngeal junction, the epiglottis and the glottic level, the coefficient of variance of four curves, the pharyngeal length, the mean cross section area and volume of pharyngeal cavity, were documented. Result: Repetitive evaluation revealed that there were not statistically significant for twice measurement within 12 hour in pharyngeal length(Z=-0.881, P=0.378), mean pharyngeal area(Z=-0.668, P=0.504) and volume (Z=-0.196, P=0.844). The area of the oropharyngeal junction (χ2=23.453, P=0.000), the epiglottis(χ2=6.632, P=0.036) and glottis(χ2=20.955, P=0.000), the coefficient of variance of four curves(χ2=7.047, P=0.029), the mean pharyngeal cross section area(χ2=26.923, P=0.000) and the pharyngeal volume(χ2=26.529, P=0.000) were statistical different between the severe OSAHS and normal subjects. There was negative correlation between body mass index(BMI), neck circumference, and the stage of disease and the area of the oropharyngeal junction, glottic area, mean pharyngeal cross section, and pharyngeal volume. Conclusion: This acoustic pharyngometry technique has good reproducibility and may provide a new clinical idea for the diagnosis and even the severity grade of OSAHS. -
